If you wish to schedule an appointment you may print out the forms located in the FORMS Section and complete. Please email all forms to kimshaffer4@gmail.com or mail to me directly: 2025 Glenwood Ave Hermon, ME 04401. Upon receipt of your paperwork. I will call you to setup an appointment. Please note that there are several forms that must be completed and returned with all pages included.
